Crossroads Middle School, as part of Northview Public Schools, is a member of the Michigan High School Athletic Association and the West Michigan Middle School Conference.  Crossroads Middle School and the student-athletes who represent Northview follow all state and conference athletic rules including:

  • Academic eligibility – weekly eligibility checks.
  • Students who plan to participate in athletics at Crossroads Middle School must have a (1) physical dated after April 15, 2025. 
  • PRIOR TO ANY SPORTS SEASON TRYOUTS/FIRST DAY OF PRACTICE, PARENTS AND STUDENTS MUST COMPLETE ALL NECESSARY PAPERWORK IN FINAL FORMS.

Registration

Registering your student for sports is done through the FinalForms platform. If you haven't used this site before, you will need to register yourself first. Follow the instructions below for registering yourself, and then your student.

Creating Your FinalForms Account

How do I sign up?

  1. Go to https://nvps-mi.finalforms.com/
  2. Click New Account under the Parent section.
  3. Fill out the fields required (Your name, date of birth and email address) and click Register.
  4. Check your email for an account confirmation email from FinalForms. Click Confirm Account in the email body.
    • NOTE: You should receive an email within 2 minutes prompting you to confirm and complete your registration. You you do not receive an email, then check your spam folder. If you still cannot locate the FinalForms email, then email support@finalforms.com and inform them of the issue.​​​​​​
  5. Create your new FinalForms password. Click Confirm Account.
  6. Click Register Student to start registering your first child.

Registering Your Student(s)

What information will I need?

Basic medical history and health information. Insurance company and policy number. Doctor, dentist, and medical specialist contact information. Hospital preference and contact information.

How do I register a student?

  1. Go to https://nvps-mi.finalforms.com/
  2. Click Login under the Parent section.
  3. Click Add Student.
  4. Type in their Legal Name and other required information. Then, click Create Student. It's important that you use your student's NVPS email address when registering.
  5. If your student plans to participate in a sport, activity, or club, then click the checkbox for each interest. Click Update after making your selection. Selections may be changed until the registration deadline. Checking the box does not obligate you but ensures you receive any information sent out by the coach prior to the season.
  6. Complete each form and sign your full name in the parent signature field on each page. After signing each, click Submit Form and move on to the next form.
  7. When all forms are complete, you will see a "Forms Finished" message. Be sure to keep an eye out in your email for any additional forms your student may need to sign.

How do I register additional students?

Click My Students then repeat steps 3 - 7 above.

How do I update information?

Login at any time and click Update Forms to update information for any student.
